Responsibilities
- Complete 80-hour paid training program to become a certified Health Advocate
- Conduct community outreach and education sessions on preventive health topics
- Assist patients with navigating healthcare systems and insurance processes
- Collect and maintain confidential health data using our digital platform
- Collaborate with local clinics and social service agencies on resource referrals
- Participate in bi-weekly case reviews and continuing education sessions
- Represent the organization at community health fairs and informational events
Qualifications
- High school diploma or equivalent (GED)
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer proficiency with willingness to learn new systems
- Reliable transportation for community visits (mileage reimbursed)
- Pass background check and drug screening
- Commitment to serving diverse populations with empathy and respect
- Flexible schedule availability including some evenings/weekends
